Mission:
To strengthen community health systems and empower vulnerable populations through integrated health, skilling, and livelihood programs that promote dignity, equity, and sustainability.
In 2023 Ismail joined Idealist.org’s mentorship program to learn how to create sustainable impact in communities facing long distances to health facilities, self-medication, and high healthcare costs. He mobilized a team—3 nurses, 2 social workers, 1 clinical officer, and 1 additional social worker—to launch the CHP medical clinic, providing diagnosis, treatment, and free or low-cost care for women, mothers, and children who could not afford private services or travel to distant public facilities. Early clinical work was financially demanding; initial drug funding came from Dr. Roopal Patel. By late 2024 CHP expanded from clinical services to community outreach focused on disease prevention (malaria, TB, HIV, diarrhea), prevention of gender- and domestic-based violence, sexual and reproductive health, and adherence support for people on HIV/TB treatment.
To address root causes of vulnerability, CHP introduced skilling and mentorship programs for adolescent girls and young women and integrated health, skills development, and livelihoods to strengthen community health systems. CHP is officially registered in Uganda as a Community-Based Organization, accredited by the Uganda AIDS Commission, and licensed by the Allied Health Practitioners Council of Uganda to operate a medical clinic. A Board of Advisors provides governance, strategic direction, transparency, and accountability.
